Martha: Support that changed a life

Martha had always received some NDIS support while living with her family in Melbourne, however her initial plan of $45,379.99 was underutilised, resulting in limited opportunities to build her capacity. Upon relocating to Cobram, VIC, with her brother and sister-in-law, they turned to Ohana for support. Our mission was clear: to understand Martha holistically and empower Martha and her family by providing the tools and knowledge necessary to truly unlock her potential.

Our first priority was a thorough review of her NDIS Plan. We carefully introduced providers who not only addressed Maratha’s immediate needs but also aligned with her long-term goals. Our team worked diligently to ensure Martha had access to the services and resources that would facilitate her growth, focusing on providers who genuinely cared about enhancing her capacity. The result? A truly transformative plan upgrade, giving her access to $313,287.65 in NDIS funding.

Martha described this opportunity as “life-changing.” Soon, her aspirations for living independently and participating in daily activities she had never previously experienced began to take shape. In 2022, with Ohana by her side, Martha embarked on a new chapter, by moving into her Supported Independent Living (SIL) home. Today, she considers her Support Coordinator as part of her family.

Caleb: Aged care to successful independence

Caleb spent several months in aged care, confronting the challenges associated with his physical limitations while striving for independence. 

With Ohana’s engagement and a dedicated support team led by Caleb, a person-centred plan was implemented to facilitate his goal of regaining independence and ensuring a smooth discharge to assist him in returning home. Ohana and Caleb’s support team recognised the significant challenges posed by limited NDIS funding, which affected his goals, daily activities, and the ongoing assistance required to navigate these obstacles. With Ohana’s specialist support, we were able to secure the necessary resources to help Caleb achieve his objective of building independence, culminating in a total funding allocation of $520,945.36.

Caleb’s journey serves as a powerful testament to the effectiveness of person-centred support, demonstrating that individuals can reclaim their independence and flourish.  

Phil: Hospital to home; a brighter future

When Ohana first engaged with Phil through a call from the Finley Hospital, we promptly engaged his NDIS planner and the hospital social worker to ensure that his first plan would facilitate more than just a transition beyond a quick discharge. Phil’s disability had a profound impact on his independence, particularly as he resided in a small, remote country town in New South Wales. Our first step? Understand Phil. What did Phil want? How did Phil envision his future and how could we help him pave that path forward?  

Leveraging our expertise in hospital-to-home transitions, we collaborated closely with the hospital team and Specialist Disability Accommodation (SDA) providers to guarantee a smooth move into a suitable facility. Acknowledging the complexities and timelines of this journey, we enlisted dedicated support workers to help Phil engage with his community and enhance his capabilities in social settings. Behind the scenes, Phil's support team strived to comprehend the challenges linked to his diagnosis, implementing thoughtful strategies whilst ensuring that the cherished memories of his rural life remained a vibrant part of his new chapter.

In a heartfelt effort to ease Phil's transition, our Support Coordinator worked closely with his support workers to gather personal belongings cultivating meaning and memories from his rural life, ensuring that his new environment felt familiar and comforting. 

Through continuous collaboration and unwavering dedication, Phil’s team not only facilitated his transition but also empowered him to thrive in his new home. As days turned into weeks, Phil began to adapt and embrace his new life, finding joy in new friendships and comfort in the familiar touches of his past.
